This form should be utilized by both Peoria Medical Student Council members (PMSC) and non-PMSC members to request funds from PMSC to fund activities and events during the academic year.

You must submit your funding request by the third Tuesday of the month so it can be reviewed/approved by the Budget Advisory Committee (BAC).
